How To Transfer Your Rx
DISCOVER A NEW LEVEL OF CARE
New customers can transfer their Rx to Miraya Pharmacy from another pharmacy by providing their personal details. So our team can verify who you are with your old pharmacy.
Email or Fax your info:
- Full name, Phone number, Date of birth
- Your old pharmacy name and phone number
- Add the medication name and Rx number for all the medications you’d like to transfer. Otherwise, state “Transfer all of my medications”.
- Verify your insurance by email or in-store.
- Subject of the email/fax should be “Transfer Rx”.
